Club Championships and Top Form Trophy Entry Forms

See below for entry forms for the Club Championships and Top Form Trophy. For those people that are new to the club entry forms have to be completed in order that we can have a list of swimmers that can then be seeded to allow the event to run smoothly.  Entries need to be received by Sunday 8th January at 6pm they can either be submitted to the club desk at Lifestyles or you can e-mail Andrea Mein the entry forms.  Lindsay the head coach would like as many swimmers as possible to enter.

Please note the 200m/400m Freestyle and 100/200 Individual Medley events have just two age brackets; 11 and under or 12 and over – don’t forget to enter them too!

The Top Form Trophy has no age brackets.
